TO RECEIVE YOUR PET ID TAG VIA MAIL:

1. Purchase any (2) TWO ARM & HAMMER™ Clumping Cat Litters before 12/31/13 or use your special voucherSpecial vouchers are sometimes issued which can be used instead of cash register receipts..

2. Circle the cat litter and corresponding prices on your original cash register receipt(s). (Not required when using a special voucherSpecial vouchers are sometimes issued which can be used instead of cash register receipts..)

3. Follow instructions for completing your order form on the right, and press the Register My Order button.

4. Optionally answer a few survey questions.

5. Print the order form on your printer using the "Print my Order Form" button.

6. Mail the completed Order Form showing your order number, and include your sales receipt(s) or your special voucherSpecial vouchers are sometimes issued which can be used instead of cash register receipts. to:
