TO RECEIVE YOUR PET ID TAG VIA MAIL:

1. Purchase any (2) TWO ARM & HAMMER® Clumping Cat Litters between 4/1/10 and 12/31/10.

2. Circle the cat litter and corresponding prices on your original cash register receipt(s).

3. Fill out the form on the right using your computer, and press the Register and Print My Order Form button. Your order form will be displayed.

4. Print the order form on your printer using the "Print my Order Form" button.

5. Mail: the printed Official Mail-In form, original dated sales receipt(s) with item descriptions and product purchase prices circled, in a stamped envelope to:
